Job Summary:
Provides specialized evaluation and rehabilitation services for patients with communication, cognitive, or swallowing disorders. Develops and implements treatment plans aligned with regulatory standards and evidence-based practices to improve patient outcomes.

Responsibilities:

• Conduct comprehensive assessments using standardized tools to identify patient needs.
• Develop and implement individualized treatment plans based on patient evaluations.
• Monitor patient progress and modify treatment approaches accordingly.
• Document all clinical activities, outcomes, and interventions in electronic health records.
• Educate and counsel patients, families, and caregivers on strategies to manage speech and swallowing disorders.
• Collaborate effectively with healthcare team members regarding patient status and care plans.

Qualifications:

• Hold current state licensure as a Speech-Language Pathologist.
• Possess a CCC-SLP certification from ASHA.
• Have at least 1 year of clinical experience as an SLP; new graduates with Level II experience accepted.
• Demonstrate knowledge of regulatory requirements and documentation standards.
• Experience in LTC or rehab settings preferred.
