Job Description
Job Summary:
Provides assessment, treatment, and rehabilitation services for patients with communication, cognitive, or swallowing disorders. Develops individualized care plans to improve patient outcomes while adhering to regulatory and clinical standards.
Responsibilities:
- Evaluate patients' communication, cognitive, and swallowing functions using standardized assessments.
- Develop and implement evidence-based treatment plans tailored to patient needs.
- Monitor and document patient progress, adjusting treatments as needed in compliance with regulations.
- Educate patients, families, caregivers, and other healthcare providers on speech and language disorders and management strategies.
- Communicate effectively with the healthcare team regarding patient progress and treatment adjustments.
- Complete comprehensive discharge summaries and maintain accurate documentation within electronic health records.
Qualifications:
- Hold a current State Licensure as a Speech-Language Pathologist.
- Possess active ASHA Certification (CCC-SLP).
- Have at least 1 year of recent SLP experience; new grads with Level II experience may be considered.
- Demonstrate minimum 3 years of experience with active CCS (preferred).
- Be able to supervise and support CFY or Student Speech-Language Pathologists as needed.
