Job Summary:

Seeking a dedicated Speech Language Pathologist to provide assessment and therapy for high-acuity patients within a skilled nursing facility. The role includes light caseload responsibilities and participation in interdisciplinary team activities to enhance patient outcomes.

Responsibilities:

• Lead and coordinate therapy services with the rehabilitation team, including PT, OT, and other speech-language pathologists.
• Oversee compliance with PDPM processes to ensure accurate documentation and reimbursement.
• Develop and implement individualized treatment plans tailored to high-acuity patients.
• Attend facility meetings, coordinate scheduling, and contribute to multidisciplinary collaboration.
• Monitor clinical and performance metrics to ensure quality standards are met.
• Ensure adherence to federal, state, and facility regulations governing therapy services.

Qualifications:

• Minimum of 1 year of Speech-Language Pathology experience in a SNF/LTC setting.
• Proven leadership experience within a rehabilitation program.
• In-depth knowledge of PDPM guidelines and documentation.
• Valid Missouri State License as a Speech-Language Pathologist.
• New graduates are not eligible for this position.
