Job Summary:
Provides supportive therapy services under the supervision of a licensed therapist in skilled nursing facilities. Assists patients with rehabilitation activities to improve their functional abilities and independence.

Responsibilities:

• Implement established treatment plans to aid patient recovery and progress.
• Monitor and document patient responses and progress during therapy sessions.
• Collaborate with the therapy team to modify interventions as needed.
• Maintain accurate and timely documentation within the rehab software system.
• Adhere to all safety and clinical protocols to ensure patient well-being.

Qualifications:

• Hold an active CA license for Occupational Therapy Assistant.
• Have SNF (Skilled Nursing Facility) experience.
• Demonstrate knowledge of Rehab Optima (NetHealth) electronic medical record systems.
• Possess strong communication and teamwork skills.
• Be committed to providing compassionate patient care.
