Job Summary:
Seeking a qualified Registered Nurse to provide exceptional patient care in a long-term care or skilled nursing facility setting. This position requires professionalism, attention to detail, and the ability to adapt to on-call responsibilities.

Responsibilities:

• Provide direct patient care according to physician’s orders and facility protocols.
• Assess patient conditions and accurately document observations and treatments.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Ensure compliance with safety standards, including ID badge requirements and infection control policies.
• Participate in patient and family education to promote health and well-being.
• Maintain confidentiality and adhere to documentation standards, especially regarding vaccine status and exemptions.

Qualifications:

• Valid state RN license in good standing.
• Proven experience in LTC or SNF settings preferred.
• Ability to demonstrate proficiency in patient assessment and care planning.
• Strong communication and documentation skills.
• Must bring a valid form of government-issued ID and wear a photo badge at all times.
• Must be fully vaccinated against COVID-19 or possess a documented exemption.
