Job Summary:
Provide skilled home healthcare services to geriatric patients within the Baltimore County area, delivering patient-centered care in a home setting. Ensure clinical assessments, documentation, and treatment meet facility standards and regulatory requirements.

Responsibilities:

• Administer comprehensive nursing care to patients during scheduled home visits.
• Complete and submit accurate visit notes, assessments, and documentation within 24–48 hours.
• Perform clinical tasks such as wound care, IV therapy, and management of feeding tubes or central lines.
• Conduct initial assessments, recertifications, and transfers, adhering to OASIS documentation standards.
• Educate patients and caregivers on disease management, medication, and treatment plans.
• Maintain compliance with safety protocols, including proper charting using WellSky/Kinnser system.

Qualifications:

• Strongly prefer 2–3 years of recent skilled home health nursing experience.
• Require Maryland or compact RN license.
• Require OASIS documentation and clinical assessment experience.
• Must have BLS certification and a valid driver's license with a reliable vehicle.
• Demonstrate proficiency with wound care, IV therapy, and patient education.
