Job Summary:
Seeking an experienced Home Health Registered Nurse to provide skilled nursing care to geriatric patients within Baltimore County, with support for Baltimore City as needed. The role involves conducting patient assessments, administering treatments, and ensuring quality documentation in a home health setting.

Responsibilities:

• Perform daily home health visits, including Start of Care, recertification, routine visits, and transfers.
• Complete OASIS documentation and clinical assessments accurately and within designated timeframes.
• Administer wound care, pressure ulcer management, and wound VAC therapies.
• Manage IV therapy, home infusion, central line/port care, and phlebotomy.
• Provide medication administration, pain management, and blood glucose monitoring.
• Document visit notes and submit paperwork within 24–48 hours of each visit.

Qualifications:

• Hold a valid Maryland or compact RN license.
• Possess 2–3 years of recent skilled home care experience, including OASIS documentation.
• Demonstrate experience with wound care, IV therapy, and medication management.
• Be BLS certified and have a valid driver’s license with reliable transportation.
• Be proficient in using WellSky/Kinnser charting system.
• Possess strong clinical assessment skills and the ability to work independently.
