Job Summary:
Provides specialized physical therapy services to patients in accordance with physician orders and established protocols. Responsible for evaluating patient needs, developing treatment plans, and educating patients and their families on therapy processes.

Responsibilities:

• Assess patient function through formal and informal testing to determine skeletal, neurological, respiratory, and cardiovascular status.
• Develop goal-specific treatment plans based on assessment data and patient tolerance.
• Submit and update treatment regimes, coordinate with physicians, and co-sign orders as necessary.
• Design a treatment schedule tailored to patient needs, coordinating with clinical support staff and caregivers.
• Educate patients, families, and caregivers on pathology, treatment options, prognosis, and intervention rationales.

Qualifications:

• Possess active Michigan Physical Therapist license.
• Hold current BLS certification from the American Heart Association.
• Demonstrate a minimum of one year of pertinent outpatient physical therapy experience.
• Provide current skills checklist relevant to physical therapy.
• Submit two supervisory reference forms completed within the past two years.
