Seeking a dedicated Physical Therapist to provide comprehensive rehabilitation services in a skilled nursing facility setting. The role involves developing and implementing individualized treatment plans to improve patient mobility and function.

Responsibilities:

• Evaluate patients’ physical conditions and establish personalized treatment goals.
• Develop and modify therapeutic interventions based on patient progress.
• Educate patients and their families on therapeutic exercises and wellness strategies.
• Collaborate with healthcare team members to ensure cohesive patient care.
• Document patient assessments, treatment plans, and progress notes accurately.
• Follow all regulatory and safety standards related to patient care.

Qualifications:

• Current and valid Physical Therapist license.
• Proven experience working in a skilled nursing facility (SNF) setting preferred.
• Strong knowledge of rehabilitation techniques and modalities.
