Job Summary:
We are seeking a qualified Physical Therapy Assistant to support patient care in a skilled nursing facility. The ideal candidate will have experience in SNF settings and familiarity with NetHealth (Rehab Optima). Current active CA license is required to ensure compliance and licensing standards.

Responsibilities:

• Assist licensed physical therapists in developing and implementing treatment plans
• Administer therapeutic exercises and modalities to patients based on established protocols
• Document patient progress and communicate effectively with the healthcare team
• Maintain accurate records ensuring compliance with regulatory and facility standards
• Support patient mobility and functional independence during therapy sessions
• Adhere to safety and infection control protocols to ensure a safe care environment

Qualifications:

• Valid and current California Physical Therapy Assistant license
• Experience working in skilled nursing facilities (SNF preferred)
• Knowledge of NetHealth (Rehab Optima) or similar rehabilitation software
• Strong communication and documentation skills
• Ability to follow treatment plans and adapt to patient needs effectively
• Professional licensure in good standing with relevant state regulatory agencies
