Job Summary:
The Outreach Medical Assistant supports population health initiatives by conducting patient outreach to improve healthcare outcomes and close quality care gaps. This role focuses on engaging patients via calls, emails, and mail, facilitating access to preventive and chronic care services, and coordinating care activities within the healthcare team.

Responsibilities:

• Execute outbound outreach calls to assist patients with scheduling necessary medical services.
• Engage with patients empathetically to understand barriers and provide guidance on care options.
• Collaborate with care teams to coordinate appointments and close identified care gaps promptly.
• Document all patient interactions and outreach efforts accurately within the electronic health record system.
• Generate reports to track outreach outcomes and evaluate intervention effectiveness.
• Assist in re-engaging disengaged patients and high-utilizer populations with appropriate care services.

Qualifications:

• Certified Medical Assistant (CMA) credential required.
• Proven experience in patient outreach, engagement, or care coordination within a healthcare setting.
• Strong communication skills, with the ability to empathetically engage diverse patient populations.
• Detail-oriented with proficiency in documenting in electronic health record systems.
• Ability to work collaboratively with healthcare teams and support staff.
• Knowledge of population health initiatives, preventive care, and chronic disease management preferred.
