Job Summary:
We are seeking a skilled Physical Therapist to provide patient-centered rehabilitation services in a Skilled Nursing Facility setting. The therapist will assess, develop, and implement individualized treatment plans to promote functional progress and independence.

Responsibilities:

• Evaluate patients’ physical status and establish treatment goals.
• Develop and modify treatment plans in collaboration with the healthcare team.
• Administer therapeutic interventions to improve mobility, strength, and overall function.
• Document patient progress and communicate effectively with team members.
• Follow facility protocols and adhere to clinical guidelines, including productivity standards.
• Maintain professional appearance and ensure compliance with dress code policies.

Qualifications:

• Valid state license as a Physical Therapist.
• Proven experience working in a SNF or similar long-term care environment preferred.
• Ability to follow treatment plans and document patient progress accurately.
• Excellent communication and interpersonal skills.
• Commitment to maintaining a professional appearance and adhering to facility policies.
• Ability to work independently and as part of a multidisciplinary team.
