Job Summary:
Provides coordination and delivery of home hospice nursing care according to physicians' orders and agency policies. Responsible for implementing the nursing process, supervising home health aides, and ensuring quality patient care in a home setting.

Responsibilities:

• Coordinate and deliver patient-centered hospice nursing care in the home environment.
• Assess patient needs, develop care plans, and adjust treatments as necessary.
• Supervise and evaluate home health aides, licensed practical nurses, and vocational nurses.
• Document patient progress, treatments, and outcomes accurately within required timeframes.
• Travel to patient homes within the designated territory, maintaining efficient scheduling.
• Comply with agency policies, documentations standards, and regulatory requirements.

Qualifications:

• Hold an active, unrestricted RN license in the state of practice.
• Have a minimum of 1 year of recent adult home health or hospice nursing experience.
• Proficient in the use of HomeCare HomeBase (HCHB) EMR system within the last 6 months.
• Demonstrate proficiency with Oasis documentation for RNs.
• Possess current CPR and BLS certifications.
• Have reliable transportation, a valid driver’s license, and agency-required liability insurance.
