Job Summary:
Provides comprehensive home nursing care to hospice patients according to physicians' orders, leveraging clinical expertise and adhering to agency policies. Coordinates and supervises patient care activities, ensuring quality service delivery within the home health setting.

Responsibilities:

• Coordinate home visits and provide patient assessments in accordance with care plans.
• Supervise and evaluate the performance of Home Health Aides and LPNs/LVNs.
• Document patient progress and care activities accurately using designated EMR systems.
• Travel up to 75 miles to deliver care at patient homes, managing a caseload of 5-8 patients daily.
• Participate in periodic office meetings to ensure care consistency and compliance.
• Adhere to all agency protocols, including on-call rotations and documentation standards.

Qualifications:

• Hold a valid RN license in good standing in the state of practice.
• Possess a minimum of 1 year of recent adult home health or hospice experience.
• Proficient in OASIS documentation and HomeCare HomeBase (HCHB) EMR system.
• Have reliable transportation, a current driver’s license, and appropriate auto insurance.
• Demonstrate strong clinical skills in hospice care and patient assessment.
• Able to travel up to 75 miles within assigned territory.
