Job Details

Job Summary:
Provide occupational therapy services to adult patients in a home health setting, including start of care and follow-up visits. Ensure patient needs are met through comprehensive assessments, interventions, and documentation.

Responsibilities:

• Conduct home visits for occupational therapy assessments, treatments, and re-evaluations.
• Develop and implement individualized care plans based on patient evaluations.
• Document patient progress and interventions accurately within required timeframes.
• Travel up to 75 miles from the clinic to meet patient care needs.
• Communicate and coordinate with healthcare team members regarding patient status.
• Maintain current licensure, certifications, and insurance requirements as mandated.

Qualifications:

• Hold a valid occupational therapist license in the state of practice.
• Minimum of 1 year of recent adult home health experience.
• Proficient in OASIS documentation with recent usage or completed retraining.
• Possess current CPR and BLS certifications.
• Demonstrate reliable transportation, driver’s license, and auto insurance.
• Willingness to travel within designated territory and accept all scheduled patients.
