Job Summary:
Assist patients in achieving their rehabilitation goals by providing skilled physical therapy support under supervision in a skilled nursing facility. Ensure quality patient care and comply with facility policies and protocols.

Responsibilities:

• Implement individualized treatment plans in accordance with physician and therapist guidelines.
• Observe, record, and report patient responses and progress.
• Utilize appropriate therapeutic techniques and equipment during sessions.
• Maintain accurate documentation and follow documentation standards.
• Adhere to facility policies, including dress code and productivity requirements.
• Support the therapy team and collaborate with staff to optimize patient outcomes.

Qualifications:

• Credentialed as a Licensed or Certified Physical Therapist Assistant.
• Proven experience providing physical therapy support in a SNF or similar setting.
• Ability to follow care plans and documentation protocols precisely.
• Strong communication and teamwork skills.
• Must comply with dress code (black scrubs) and productivity standards.
• Must be present in the building to receive guaranteed hours.
