Job Summary:
Providing comprehensive physical therapy services to patients in inpatient, swing bed, or skilled nursing facilities, with potential coverage at hospital settings as needed. Responsible for assessing patient needs, developing treatment plans, and implementing therapeutic interventions to improve patient outcomes.

Responsibilities:

• Evaluate patients’ physical conditions and develop individualized treatment plans.
• Administer therapeutic exercises and modalities according to prescribed protocols.
• Monitor and document patient progress accurately and promptly.
• Educate patients and families on therapy procedures and home programs.
• Collaborate with healthcare team members to coordinate patient care.
• Maintain compliance with all regulatory and licensing requirements.

Qualifications:

• Hold a Bachelor of Science degree in Physical Therapy.
• Possess a current California Physical Therapy License.
• Maintain valid BLS certification approved by the AHA.
• Pass the National Physical Therapist Examination (NPTE).
• Have at least 1+ year of experience in acute, swing bed, skilled nursing, or outpatient settings.
• Demonstrate excellent interpersonal communication, problem-solving, and documentation skills.
