Job Summary:

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure adherence to care plans and maintain high-quality patient care standards.

Responsibilities:

• Administer medication and treatments accurately according to care plans.
• Monitor residents’ health status and report any changes to the healthcare team.
• Document nursing assessments, interventions, and observations thoroughly.
• Follow infection control protocols and maintain a safe environment.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Comply with facility policies and state/federal regulations regarding patient care.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC or SNF settings preferred.
• Ability to document accurately and efficiently.
• Strong communication and interpersonal skills.
• Must have completed the original or updated COVID-19 vaccination or hold a valid exemption.
• Ability to wear a photo badge and present a state or federal-issued ID at all times.
