Job Summary:

Provide comprehensive physical therapy services to patients in a home health setting, ensuring optimal functional outcomes and adherence to clinical protocols. This role involves traveling to patient homes, documenting care, and collaborating with healthcare teams.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Administer therapeutic exercises and interventions to improve mobility and strength.
• Educate patients and caregivers on treatment programs and injury prevention.
• Document patient progress and clinical activities accurately within 24 hours.
• Travel to patient homes up to 75 miles away, ensuring timely and efficient visits.
• Participate in periodic office meetings and on-call rotations as required.

Qualifications:

• Valid physical therapy license in the state of practice with in-hand credentials.
• Minimum of 1 year of recent adult home health experience.
• Proficiency in OASIS documentation and familiarity with EMR systems such as HomeCare HomeBase.
• CPR and BLS certification required.
• Ability to travel up to 75 miles daily and maintain reliable transportation.
• Strong communication skills and clinical competence in home health therapy.
