Job Summary:

Provide specialized physical therapy services to patients by developing and implementing individualized treatment plans aimed at improving physical function, reducing pain, and preventing dysfunction. Collaborate with healthcare teams to deliver effective patient care in a clinical setting.

Responsibilities:

• Design, prepare, and execute personalized physical treatment programs based on patient assessments.
• Complete and document all patient evaluations, tests, and progress notes in compliance with standards and regulations.
• Develop and update the patient’s Plan of Care (POC) utilizing evaluation data, diagnosis, and clinical judgment.
• Assess and deliver therapeutic interventions in accordance with approved care plans, adjusting as needed.
• Maintain accurate documentation for charge capture and compliance purposes.
• Coordinate with multidisciplinary care team members to ensure cohesive patient management.

Qualifications:

• Graduate of an accredited Physical Therapy Program.
• Valid state licensure as a Physical Therapist.
• BLS certification from AHA or ARC.
• Minimum of 2 years of relevant outpatient orthopedic experience.
• Flexible availability, including daytime shifts Tuesday through Saturday.
• Demonstrated ability to work effectively within a team and adhere to regulatory standards.
