Job Summary:
We are seeking a skilled Physical Therapist to provide high-quality rehabilitative services to patients in a skilled nursing facility setting. The ideal candidate will evaluate patient conditions and develop personalized treatment plans to promote recovery and improve mobility.

Responsibilities:

• Assess patients’ physical needs and establish individualized care plans.
• Implement therapy techniques to improve strength, flexibility, and mobility.
• Monitor and document patient progress regularly.
• Collaborate with healthcare team to coordinate patient care.
• Educate patients and families on rehabilitation exercises and injury prevention strategies.

Qualifications:

• Hold a current state licensure as a Physical Therapist.
• Possess relevant work experience in a SNF or similar environment.
• Demonstrate strong assessment and clinical skills.
• Exhibit excellent communication and team collaboration abilities.
• Maintain CPR and BLS certifications.
