Job Summary:

Seeking a dedicated Physical Therapist to provide home health services, including patient assessments and therapeutic interventions. The role involves traveling to patient homes, managing a caseload, and ensuring quality care delivery.

Responsibilities:

• Evaluate patient conditions and develop customized treatment plans aligned with physician directives.
• Perform home visits to deliver therapeutic services, monitor progress, and adjust care plans accordingly.
• Document patient interactions and treatment outcomes accurately within the required time frame.
• Collaborate with care teams during periodic meetings and update relevant documentation.
• Maintain compliance with all licensing, documentation, and safety protocols.
• Travel up to 75 miles from the assigned clinic to reach patients in their residences.

Qualifications:

• Hold a valid state license as a Physical Therapist with current credentials in hand.
• Possess at least 1 year of recent adult home health care experience.
• Demonstrate proficiency in OASIS and EMR systems, specifically HomeCare HomeBase.
• Have reliable transportation, a valid driver’s license, and agency-required liability insurance.
• Able to travel up to 75 miles and accept patients across a wide service area.
• Maintain CPR/BLS certification as required.
