Job Summary:
Provide comprehensive physical therapy services to patients in a home health setting, focusing on assessing, planning, and implementing individualized treatment plans to improve patient mobility and function.

Responsibilities:

• Evaluate patients' physical conditions and establish treatment goals.
• Develop and implement personalized therapy plans tailored to each patient's needs.
• Conduct therapy sessions, including exercises and manual techniques.
• Document patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on exercises, mobility aids, and safety procedures.
• Comply with all documentation, scheduling, and regulatory requirements.

Qualifications:

• Hold a valid and current state licensure as a Physical Therapist.
• Have at least 1 year of recent home health or outpatient therapy experience.
• Demonstrate proficiency in OASIS documentation and EMR systems such as PointCare/HCHB.
• Possess a current chauffeur’s license and reliable transportation.
• Be able to travel up to 75 miles from the home clinic.
• Have current CPR/BLS certification.
