Job Summary:
Provides comprehensive home-based nursing care to patients, managing a patient caseload and collaborating with multidisciplinary teams to achieve optimal health outcomes. Performs assessments, educates patients and families, and ensures quality, safe care delivery in accordance with care plans.

Responsibilities:

• Conduct comprehensive initial and ongoing assessments of patients' needs at scheduled intervals.
• Administer skilled nursing interventions independently and in coordination with the care team.
• Educate patients and families on health management, medication, and safety practices.
• Document patient information accurately and timely, including care plans, responses, and progress.
• Adjust care plans based on ongoing assessments and patient response.
• Collaborate with other disciplines to ensure holistic, patient-centered care.

Qualifications:

• Hold a valid RN license in the state of practice.
• Possess a Bachelor’s Degree in Nursing (BSN) preferred.
• Have experience in acute care, home care, or LTC/SNF settings preferred.
• Maintain current CPR/BLS certification from an approved vendor.
• Possess a valid driver’s license, auto insurance, and reliable transportation.
• Be at least 21 years of age or older.
