Job Summary:
Provide specialized occupational therapy services to geriatric patients in a home health setting. Deliver individualized treatment plans to improve patient functional abilities and support independence.

Responsibilities:

• Assess patients’ functional status and develop tailored treatment plans to meet their needs.
• Implement therapeutic interventions to enhance mobility, daily living skills, and overall well-being.
• Collaborate with interdisciplinary teams to coordinate patient care and ensure compliance with regulations.
• Document patient progress accurately within electronic health records such as HCHB and Epic.
• Educate patients and family members on therapy techniques and home exercise programs.
• Maintain licensure and certifications, including BLS, and adhere to compliance standards.

Qualifications:

• Hold a valid occupational therapist license for Georgia or a Compact license.
• Possess at least 1 year of clinical experience in home health or geriatrics.
• Demonstrate proficiency with electronic health record systems such as HCHB and Epic.
• Be BLS certified with the ability to complete required online courses if needed.
• Ability to work within facility scope, float as needed, and cover assigned counties.
• Be willing to work full-time, Day shift (4x10 hours).
