Job Summary:
As a Physical Therapist at Santa Rosa Post Acute, you will assess and treat residents to restore physical function and enhance mobility. You will develop individualized treatment plans and collaborate with a multidisciplinary team to promote residents' independence and overall well-being.

Responsibilities:

• Conduct comprehensive evaluations of patients' physical abilities and tailor personalized treatment plans.
• Implement therapeutic interventions aimed at improving strength, mobility, and functional skills.
• Educate patients and families on exercises, safe movement practices, and home modifications.
• Collaborate with occupational therapists, nurses, and physicians to coordinate comprehensive patient care.
• Track patient progress and modify treatment strategies to achieve optimal outcomes.
• Document assessments, treatment plans, and progress notes accurately and promptly, ensuring compliance with regulations.

Qualifications:

• Possess a Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited institution.
• Hold an active Physical Therapy license in the state.
• Experienced in skilled nursing facilities or comparable healthcare environments preferred.
• Skilled in physical therapy techniques, modalities, and adaptive equipment usage.
• Demonstrate excellent communication, interpersonal, and problem-solving skills.
• Capable of working independently and within team settings with a compassionate and patient-centered mindset.
