Job Summary:
Seeking a dedicated Physical Therapist to deliver high-quality outpatient and home health therapy services. The ideal candidate will have experience in Oasis documentation and outpatient care, with a strong focus on patient recovery and rehabilitation.

Responsibilities:

• Evaluate patients' physical conditions and develop personalized treatment plans.
• Administer therapeutic interventions to improve mobility, strength, and function.
• Complete Oasis documentation accurately and in a timely manner.
• Collaborate with healthcare team members to coordinate patient care.
• Collect and monitor patient progress, adjusting treatment plans as needed.
• Educate patients and caregivers on exercises and home programs to promote recovery.

Qualifications:

• Hold an active CA Physical Therapy license or be eligible to apply.
• Possess at least 1 year of home health or outpatient therapy experience.
• Demonstrate proficiency in Oasis documentation.
• Maintain current BLS certification (AHA preferred).
• Have a valid driver's license and reliable transportation.
• Show excellent communication and documentation skills.
