Job Summary:
A licensed Physical Therapist is responsible for developing and implementing individualized treatment plans to improve patient mobility and function in a Skilled Nursing Facility setting. They work closely with patients and healthcare teams to promote recovery and enhance quality of life.

Responsibilities:

• Assess patients’ physical conditions and determine therapy needs.
• Develop and modify personalized treatment plans based on patient progress.
• Deliver therapeutic interventions to improve strength, mobility, and coordination.
• Educate patients and their families on treatment goals and exercises.
• Document patient progress and maintain accurate clinical records.
• Collaborate with healthcare professionals to coordinate care and ensure effective treatment delivery.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess a bachelor’s or master’s degree in Physical Therapy from an accredited program.
• Demonstrate excellent communication and interpersonal skills.
• Exhibit strong clinical assessment and problem-solving abilities.
• Maintain current CPR and BLS certifications.
• Prior experience in a skilled nursing or rehab setting preferred.
