Job Summary:
Seeking an experienced Registered Nurse specialized in Home Health to perform comprehensive in-home assessments and collaborate with interdisciplinary teams. The nurse will evaluate patient health status, develop care plans, and ensure quality care delivery within a fast-paced environment.

Responsibilities:

• Conduct in-home pre-evaluations to assess health status, strengths, care needs, and patient preferences.
• Review and analyze medical documentation and health referral forms related to each case.
• Enter evaluation data into electronic forms and transmit information accurately and timely.
• Document concerns, discrepancies, or issues identified during assessments to ensure accurate records.
• Facilitate continuity of care by coordinating with care teams and minimizing treatment fragmentation.
• Communicate with consumers regarding healthcare options, services, and waiver programs to support informed decision-making.

Qualifications:

• Hold current RN licensure in New York State.
• Possess a minimum of two (2) years recent experience in a home setting within the last ten (10) years.
• Demonstrate the ability to perform comprehensive assessments similar to UAS or OASIS evaluations.
• Have experience working with geriatric populations managing chronic diseases in a home environment.
• Exhibit strong organizational, interpersonal, and communication skills, both verbal and written.
• Ability to work independently in a fast-paced, deadline-driven setting with technological proficiency.
