Job Summary:
The RN Nurse Assessor is responsible for performing comprehensive in-home assessments to evaluate patient health status and care needs. They will review medical documentation, develop individualized care plans, and collaborate with interdisciplinary teams to ensure quality care delivery.

Responsibilities:

• Conduct initial in-home evaluations assessing health status, strengths, and care requirements.
• Review medical records and health referral documentation for accuracy and completeness.
• Record evaluation findings accurately into electronic systems and transmit data as required.
• Communicate with consumers regarding healthcare options, support services, and waiver opportunities.
• Collaborate with care teams and community resources to coordinate services and improve patient outcomes.
• Document concerns, conflicting information, and relevant issues identified during assessments.

Qualifications:

• Current RN licensure in New York State.
• At least two years of recent experience working in a home health setting within the last ten years.
• Experience conducting assessments similar to UAS or OASIS, preferably in NY (role 40 or 45).
• Demonstrated ability to work independently in a fast-paced, deadline-driven environment.
• Strong organizational, communication, and interpersonal skills.
• Experience managing chronic diseases in geriatric populations, especially in-home care.
