Job Summary:

We are seeking a dedicated and skilled Physical Therapist to provide patient-centered rehabilitation services across acute inpatient and skilled nursing settings. The role offers the opportunity to work with diverse patient populations, contributing to improved functional outcomes and recovery.

Responsibilities:

• Evaluate patients’ physical conditions to develop personalized treatment plans.
• Deliver evidence-based therapeutic interventions for neurological and geriatric rehabilitation.
• Collaborate with multidisciplinary teams to coordinate care and optimize patient progress.
• Document patient assessments, progress notes, and treatment outcomes accurately.
• Maintain compliance with all licensure, certification, and facility policies.
• Adapt therapies based on patient response and evolving clinical needs.

Qualifications:

• Hold a Doctor of Physical Therapy (DPT) degree from an accredited program.
• Possess an active state licensure as a Physical Therapist or be eligible for licensure.
• Demonstrate experience in neurological rehabilitation or acute inpatient care preferred.
• Show proficiency in documentation, organization, and interdisciplinary collaboration.
• Maintain current CPR/BLS certification and California PT license.
• Exhibit excellent communication and problem-solving skills.
