Job Summary:

Physical Therapist provides therapeutic services to patients in a skilled nursing setting, helping restore function, improve mobility, manage pain, and support independence. This role collaborates with the care team to develop and implement individualized treatment plans based on patient needs.

Responsibilities:

• Assess patients’ physical condition, mobility, strength, balance, and functional abilities.
• Develop, implement, and update individualized treatment plans to achieve measurable goals.
• Provide therapeutic exercise, manual therapy, neuromuscular re-education, and gait training.
• Evaluate treatment outcomes and adjust interventions based on progress and clinical findings.
• Educate patients and caregivers on safe movement, home exercise programs, and fall prevention.
• Coordinate with interdisciplinary staff to support comprehensive patient care and documentation.

Qualifications:

• Hold an active Physical Therapist license (or applicable state licensure) in good standing.
• Demonstrate knowledge of PT evaluation, treatment planning, and outcome measurement.
• Exhibit strong clinical judgment in delivering patient-centered, evidence-based care.
• Maintain current CPR certification as required by policy or licensure standards.
• Communicate clearly with patients, families, and interdisciplinary team members.
• Document clinical findings, treatments, and progress in accordance with regulatory and facility requirements.
