Job Summary:
Dedicated Physical Therapist responsible for developing and implementing rehabilitation programs to improve patients' physical functions. Uses advanced therapeutic techniques and compassionate care to support patient recovery and well-being.

Responsibilities:

• Design and execute personalized rehabilitation plans tailored to patient needs.
• Utilize therapeutic exercises, neuromuscular re-education, and manual techniques to enhance mobility.
• Assess patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on therapeutic exercises and injury prevention.
• Maintain accurate documentation of treatments and patient responses.
• Collaborate with healthcare team members to optimize patient outcomes.

Qualifications:

• Hold an active Illinois Physical Therapist license.
• Possess a minimum of 1 year of relevant PT experience.
• Demonstrate proficiency in therapeutic exercises, gait training, and physical modalities.
• Have current BLS certification through the American Heart Association.
• Provide two supervisory reference forms with employment dates.
• Must reside outside a 50-mile radius of the facility location.
