Job Summary:

Physical Therapist will evaluate patients and deliver skilled rehabilitation services in the home setting, driving functional improvement and supporting long-term independence. The role includes establishing individualized plans of care, coordinating interdisciplinary services, and educating patients and families.

Responsibilities:

• Evaluate patients in the home environment and establish a plan of care aligned with clinical needs.
• Deliver direct therapy interventions based on physician orders and the established treatment plan.
• Reassess patient progress regularly and adjust treatment frequency, duration, and goals as needed.
• Coordinate care with physicians and other care team members to ensure continuity and appropriate services.
• Document all skilled services accurately and timely according to clinical and regulatory standards.
• Provide supervision and oversight for Physical Therapist Assistant services when applicable.

Qualifications:

• Hold a BS degree or higher from an accredited Physical Therapy program.
• Maintain a current Physical Therapist license to practice in the applicable state.
• Possess a valid driver’s license and current, appropriate auto insurance.
• Demonstrate strong clinical judgment in home-based assessment and rehabilitation planning.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Bring at least 2 years of prior PT experience (preferred) with experience in rehabilitation settings.
