Job Summary:
Provide skilled physical therapy services to residents, promoting mobility, function, and overall well-being. Ensure compliance with all clinical standards and deliver compassionate patient care in a long-term care setting.

Responsibilities:

• Assess patients' physical condition and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and coordination.
• Document patient progress and adjust treatment plans as needed.
• Collaborate with healthcare team to ensure comprehensive care delivery.
• Educate patients and staff on therapeutic exercises and safety protocols.
• Maintain accurate records in accordance with regulatory requirements.

Qualifications:

• Hold a valid state Physical Therapist license.
• Possess recent experience in outpatient, long-term care, or rehabilitation settings.
• Demonstrate strong interpersonal and communication skills.
• Exhibit ability to work independently and within a team.
• Maintain current CPR certification.
