Job Summary:

The Physical Therapist will provide skilled physical therapy services in a home health setting, delivering individualized treatment plans that support patient mobility, function, and independence. This role requires safe, compliant patient care while managing a consistent daily visit workload.

Responsibilities:

• Deliver evidence-based physical therapy evaluations and ongoing treatment for home-based patients
• Develop and implement individualized plans of care to meet functional goals
• Perform therapeutic exercises, neuromuscular re-education, gait training, and mobility interventions
• Educate patients and caregivers on safe movement, home exercise programs, and fall prevention
• Coordinate visit documentation and clinical updates in accordance with applicable standards
• Maintain safe travel practices and adhere to required compliance protocols during patient visits

Qualifications:

• Hold an active CA Physical Therapist (CA PT) license
• Possess at least 1 year of experience as a Home Health PT
• Maintain a valid CA driver’s license and carry auto insurance
• Hold current BLS certification (preferred) compliant with AHA or ARC
• Demonstrate proficiency with clinical documentation and e-learning module completion requirements
• Provide verifiable work history and complete onboarding and background/licensure checks as required
