Job Summary:

The Physical Therapist will provide high-quality, patient-centered rehabilitation services across an Acute Inpatient Unit and a Skilled Nursing Facility setting. This role focuses on improving functional outcomes through evidence-based therapy, collaboration with interdisciplinary teams, and accurate clinical documentation.

Responsibilities:

• Evaluate patients’ physical status and functional needs to establish individualized treatment plans.
• Deliver skilled physical therapy interventions for neurological recovery and post-acute rehabilitation.
• Coordinate care with physicians, nursing, and therapy teams to support safe, goal-oriented outcomes.
• Document assessments, treatment provided, and patient progress in accordance with clinical standards.
• Maintain professional communication with patients, families, and team members to support education and adherence.
• Adapt treatment strategies to meet changing needs across acute and skilled nursing environments.

Qualifications:

• Hold a Doctor of Physical Therapy (DPT) degree from an accredited program.
• Maintain active state licensure as a Physical Therapist (or be eligible for licensure).
• Possess current CPR/BLS certification.
• Demonstrate strong knowledge of neurological disorders and evidence-based rehabilitation techniques.
• Experience with neurological rehabilitation or acute inpatient care preferred.
• Experience with geriatric rehabilitation and SNF care preferred; work effectively as part of an interdisciplinary team.
