Job Summary:
Provide occupational therapy services to patients in a home health setting, helping them regain independence and improve their daily functioning. The therapist will evaluate patients, develop treatment plans, and deliver customized interventions.

Responsibilities:

• Conduct comprehensive assessments of patients' functional abilities and limitations.
• Develop and implement individualized treatment plans based on assessment findings.
• Document patient progress and maintain accurate clinical records.
• Collaborate with caregivers and team members to ensure effective therapy delivery.
• Participate in on-call rotation and maintain timely communication with team.
• Adhere to all regulatory and agency standards, including documentation and safety protocols.

Qualifications:

• Hold a current CA Occupational Therapist license in good standing.
• Possess a minimum of 1 year of home health experience.
• Demonstrate proficiency in OASIS documentation and assessments.
• Maintain active CPR/BLS certification (AHA preferred).
• Have a valid driver’s license and reliable personal transportation.
• Be able to work independently and communicate effectively.
