Job Summary:

The Nurse Care Coordinator is responsible for providing comprehensive patient care coordination, ensuring effective communication between patients, families, and healthcare teams. This role focuses on patient advocacy, care planning, and optimizing health outcomes through collaborative efforts.

Responsibilities:

• Develop and implement individualized care plans based on patient assessments and clinical data.
• Coordinate multidisciplinary team activities to ensure seamless patient care transitions.
• Educate patients and families about health management, medication adherence, and treatment options.
• Monitor patient progress and adjust care plans accordingly to meet evolving needs.
• Document care activities and clinical observations accurately in electronic health records.

Qualifications:

• Hold a valid Registered Nurse (RN) license in the state of practice.
• Possess a minimum of three years of clinical nursing experience, preferably in acute or chronic care settings.
• Demonstrate strong communication and interpersonal skills for effective patient engagement.
• Proficient in coordinating care and managing diverse patient populations.
• Experience with electronic medical records (EMR) systems and care coordination tools.
• Ability to work independently and in team environments, demonstrating flexibility and problem-solving skills.
