Job Summary:
Seeking a skilled Nurse Care Coordinator with extensive clinical experience to manage patient care, coordinate services, and facilitate discharge planning. The role requires collaborative communication with healthcare teams and adherence to regulatory standards to ensure quality patient outcomes.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Coordinate with interdisciplinary teams to optimize patient care and facilitate efficient discharge planning.
• Review utilization and ensure appropriate resource allocation.
• Document patient progress and clinical interventions accurately in electronic health records.
• Provide guidance and support to patients and families regarding care processes and health management.

Qualifications:

• Hold an active RN license in the State of New York.
• At least five (5) years of clinical nursing experience.
• Possess proficiency in care coordination, utilization review, and discharge planning.
• Demonstrate computer literacy and familiarity with electronic health records.
• Obtain PRI and Screen certification within three months of employment.
• Credentialed with BLS certification; preferred case management certification.
