Job Summary:
Provides specialized physical therapy services to patients, assessing their functional status and developing individualized treatment plans. Delegates tasks to assistants and aides while ensuring compliance with medical guidelines and policies to promote recovery and improve patient outcomes.

Responsibilities:

• Evaluate patients' physical, neurological, respiratory, and cardiovascular functions through formal and informal testing measures.
• Develop goal-oriented treatment regimes based on assessment data and patient tolerance, implementing plans to facilitate rehabilitation.
• Submit and update treatment plans in accordance with physician orders, ensuring coordination and accuracy; cosign verbal orders as needed.
• Create treatment schedules tailored to patient needs, coordinating with patients, families, and clinical staff to optimize care delivery.
• Educate patients, families, and caregivers on their condition and treatment options, explaining pathology, prognosis, and interventions utilized.

Qualifications:

• Hold an active Michigan Physical Therapist License.
• Possess current BLS certification from the American Heart Association.
• Demonstrate at least one year of pertinent outpatient physical therapy experience.
• Provide two supervisory reference forms completed within the past two years.
• Exhibit strong assessment, communication, and patient education skills.
