Job Summary:
Seeking a Speech Language Pathologist (SLP) to provide therapy services in a skilled nursing facility on a part-time basis. The candidate will evaluate and treat patients to improve communication, swallowing, and cognitive-communication skills.

Responsibilities:

• Assess patients' speech, language, and swallowing abilities to determine treatment needs.
• Develop and implement individualized treatment plans based on clinical evaluations.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Document patient progress and adjust therapy plans accordingly.
• Comply with all facility policies, including documentation and attendance requirements.
• Maintain current licensure and certifications as required.

Qualifications:

• Hold and maintain a valid state license as a Speech Language Pathologist.
• Possess a minimum of 3 years of relevant clinical experience.
• Be BLS certified.
• Demonstrate prior experience with adult populations, preferably in SNFs.
• Ability to work flexible shifts, including 8-hour and 4-hour blocks.
• Recent EPIC EMR experience preferred but not required.
