Job Summary:
Provide skilled physical therapy services to patients in a home health setting, ensuring optimal recovery and functional improvement. Deliver care during day shifts with no weekend responsibilities, maintaining high standards of patient care and compliance.

Responsibilities:

• Evaluate patients' physical condition and develop individualized treatment plans.
• Implement therapy interventions to improve mobility, strength, and functional ability.
• Document patient progress and update care plans using HCHB and OASIS systems.
• Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
• Educate patients and families on exercises and techniques for continued recovery.

Qualifications:

• Hold a valid Doctor of Physical Therapy (DPT) license.
• Possess at least 1 year of home health experience.
• Know how to navigate HCHB and OASIS documentation systems.
• Maintain AHA BLS certification.
• Demonstrate excellent communication and patient interaction skills.
