Job Summary:

The Physical Therapist will provide outpatient and home health therapy services to patients, focusing on improving mobility, function, and quality of life. They will assess patient needs, develop personalized treatment plans, and deliver therapeutic interventions in patients' homes and care facilities.

Responsibilities:

• Evaluate patients' physical conditions to establish appropriate treatment goals.
• Develop and implement individualized therapy plans tailored to each patient.
• Conduct therapy sessions at patients' homes and care facilities, ensuring optimal care delivery.
• Document patient progress, treatment provided, and any changes to treatment plans using specified EMR systems.
• Educate patients and their families on exercises, mobility techniques, and injury prevention.
• Travel up to 75 miles from the home clinic to fulfill home health visits and meet productivity targets.

Qualifications:

• Hold a valid state license as a Physical Therapist in good standing.
• Possess at least one year of recent adult home health therapy experience.
• Proficient in OASIS documentation and HomeCare HomeBase EMR within the last 6 months.
• CPR/BLS certification is required.
• Must have reliable transportation, a current driver's license, and valid liability insurance.
• Ability to work independently, manage a flexible schedule, and travel as required.
