Job Summary:

Seeking a dedicated Physical Therapist to deliver outpatient and home health services within the San Bernardino/Ontario area. The clinician will evaluate, plan, and implement patient-centered treatment plans to improve mobility and function.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans based on clinical findings.
• Administer therapeutic interventions, including manual therapy, exercises, and modalities.
• Document patient progress accurately and in compliance with regulatory standards.
• Collaborate with healthcare teams to coordinate care and ensure optimal patient outcomes.
• Maintain current licensure, certifications, and stay informed on clinical best practices.
• Comply with agency policies, including travel and documentation requirements.

Qualifications:

• Hold a valid California Physical Therapist license in hand.
• Possess at least 1 year of outpatient and/or home health experience.
• Demonstrate proficiency with OASIS documentation and clinical assessments.
• Be BLS- AHA certified.
• Have personal transportation and an active driver's license.
• Be able to work day shifts and participate in on-call rotations if required.
