Job Summary:

The Physical Therapist is responsible for evaluating and treating patients to restore function and reduce disability following injury or illness. They develop and implement individualized therapy programs to help patients achieve optimal performance within their capabilities.

Responsibilities:

• Assess patients' physical conditions and develop personalized treatment plans.
• Administer medically prescribed physical therapy interventions to facilitate recovery.
• Document clinical observations, treatment progress, goals, and discharge plans accurately.
• Supervise Physical Therapy Assistants and coordinate patient care with the clinical team.
• Perform initial admission and evaluation visits for new patients.
• Notify the Clinical Team Manager of OASIS scores and other relevant progress reports within established timelines.

Qualifications:

• Hold a valid state license or credential as a licensed Physical Therapist.
• Possess at least one year of Home Health therapy experience.
• Demonstrate proficiency in evaluating and treating diverse patient conditions.
• Exhibit strong clinical documentation and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of OASIS documentation and home health protocols.
