Job Summary:

A licensed Physical Therapist is sought for home health settings to provide skilled therapy services. The role involves evaluating patient needs, developing treatment plans, and delivering personalized care to promote recovery and independence.

Responsibilities:

• Evaluate patients' physical conditions and develop individualized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, strength, and function
• Document patient progress and adjust treatment plans accordingly
• Collaborate with healthcare teams to ensure coordinated patient care
• Comply with all licensing, certification, and travel requirements

Qualifications:

• Hold a valid CA Physical Therapist license with at least 1 year of home health experience
• Possess a CA driver's license and auto insurance
• BLS certification preferred
• Demonstrate experience with Epic charting and documentation
• Be able to travel as required, with 2+ years of clinical experience
• Show valid work history verification covering the past 7 years
