Job Summary:
The Nurse Assessor in home health provides comprehensive patient evaluations, including health assessments and care planning, primarily conducting hybrid in-field and telehealth assessments. The role requires extensive knowledge of UAS criteria, community resources, and compliance with state and agency regulations.

Responsibilities:

• Perform initial in-home pre-evaluations assessing health status, strengths, care needs, and patient preferences.
• Review medical documentation and health referral forms accurately and efficiently.
• Enter evaluation data into electronic systems and transmit securely as required.
• Document concerns, conflicting information, or issues identified during assessments.
• Communicate effectively with consumers regarding healthcare options, supports, and service vendors.
• Facilitate enrollment communication between the Call Center and eligible consumers for managed care plans.

Qualifications:

• Hold current RN licensure in New York State.
• Possess at least two (2) years of recent home care experience within the last ten (10) years.
• Demonstrate experience in conducting assessments similar to UAS or OASIS, or providing primary care in home settings.
• Be proficient in working with health technology and electronic documentation systems.
• Showcase excellent communication, organizational, and interpersonal skills.
• Have experience caring for geriatric populations with chronic disease management.
