Job Summary:
The RN - Nurse Assessor is responsible for conducting comprehensive home health assessments to evaluate patients' health status, care needs, and preferences. This role involves collaborating with interdisciplinary teams to develop individualized care plans and ensuring quality and continuity of care for homebound individuals.

Responsibilities:

• Perform initial in-home evaluations assessing health status, strengths, care needs, and patient preferences.
• Review medical documentation and health referral forms to inform assessment decisions.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns, conflicting information, or issues identified during assessments.
• Communicate health care options, support services, and vendor choices to consumers.
• Collaborate with care teams and community resources to facilitate coordinated service delivery.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two years’ recent experience in a home health setting within the last ten years.
• Experience conducting assessments, including UAS and OASIS, preferably in the last 365 days.
• Proven ability to perform in-home assessments and care management for geriatric populations.
• Strong communication, organizational, and technological skills.
• Ability to work independently in a fast-paced environment, managing multiple complex tasks.
