Job Details

Job Summary:
Provide occupational therapy services to patients in a home health setting within designated territories. Deliver personalized care to improve patients' functional independence, adhering to regulatory standards and agency protocols.

Responsibilities:

• Assess patients' physical, emotional, and environmental needs to develop individualized treatment plans.
• Administer occupational therapy interventions, including evaluations, re-certifications, and discharge planning.
• Document patient progress accurately and maintain compliance with documentation standards.
• Collaborate with healthcare teams and coordinate care to ensure effective treatment delivery.
• Travel within assigned areas and occasionally outside the service zone to accommodate patient needs.
• Comply with all safety protocols and infection control policies, including handling COVID-positive and PUI patients.

Qualifications:

• Hold a current California Occupational Therapist license.
• Possess BLS certification and valid driver’s license with auto insurance and registration.
• Maintain current immunizations including TB, MMR, Varicella, HepB, and TDap.
• Demonstrate experience with home health therapy preferred, but not required.
• Meet all regulatory and agency-specific credentialing requirements.
